I clear it as part of the job. If a section is damaged or pulling off the fascia I will show you the photo and tell you, rather than quietly leaving it.

Twice a year under oaks, once a year on an open lot. Timing one of them before the summer storm season is worth more than doing both in winter.
Do you install gutter guards?
No, I clean gutters. If guards would genuinely help your situation I will say so and you can bring in someone who installs them.
